Subject: Fwd: Gentlemen - Your Flying Safety Meeting for the Week
There is an old axiom dealing with aviation accidents, especially catastrophic ones, it takes multiple wrong doings to create a major accident.
This Captain created a whole string of them, further added by the Flight Engineer contributing by actioning an Emergency Checklist item ENGINE FIRE, without bothering to inform the Captain, all at a time they needed every ounce of energy (thrust) they could get.
This is a CLASSIC case of NOBODY paying any attention to proper crew communications, known as CRM (crew resource management) AC 120-51D.
Result over 100 killed, plus the loss of a highly valued airplane. The Captain did everything he could to cause the crash: serviced the fuel tanks beyond their NORMAL capacity, which caused the CG to go beyond the AFT Limits, just for starters. Insisted that about 3,000 pounds of baggage be loaded, when the station manager clearly stated he was already over MGTOW (Max Gross takeoff weight), took off downwind and everything continued to go downhill from there.

Subject:  Chinese Carrier Operations ....


Appears the Chinese  are coming right along with their CV ops.  Probably doing a better job than the Russians.  Some items of note:

1) Looks as though they’re sticking to the tried and true – Fresnel lens, LSO, 4 wires (US is going to 3 on the newer carriers).
2) Flight deck jersey colors appear similar to ours.  Not many people on the flight deck.
3) Interesting launch hold-back system.  No catapult?
4) Roll-out on 3-wire puts the nose wheel very close to the end of the angle – 4-wire should be a real hoot!!
